Access Your Neighborhood Family Practice MyChart Account
Before You Log In
Many healthcare organizations use MyChart, but each organization has its own MyChart account and login.
If you are a patient of Neighborhood Family Practice, you must use your Neighborhood Family Practice MyChart account to access your health information, appointments, test results, messages, and more through this website.
Even if you have a MyChart account with another healthcare provider, hospital, or health system, you will need to use your Neighborhood Family Practice MyChart login to access your Neighborhood Family Practice records.

Signing Up for MyChart Is Easy
You can sign up for a Neighborhood Family Practice MyChart account in one of two ways:
In Person
During your next visit, ask a front desk or care team member for a MyChart activation code.
- Click the Login to MyChart button above.
- Select Sign Up Now.
- Enter your activation code and complete the registration form.
- Create a username and password.
Neighborhood Family Practice recommends using the first initial of your first name followed by your last name as your username.
Online
- Click the Login to MyChart button above.
- Select Sign Up Now.
- Complete the MyChart Request an Activation Code form and submit your request.
- A one-time activation code will be sent to you by email or mail.
- Follow the instructions provided to finish setting up your account.
Have More Than One MyChart Account?
If you receive care from multiple healthcare organizations, you may have more than one MyChart account. MyChart offers a feature called Link My Accounts, which allows you to view information from participating healthcare organizations using a single login.
How to Link Your Accounts
- Log in to your Neighborhood Family Practice MyChart account.
- Select Menu.
- Under the Sharing section, choose Link My Accounts.
- Search for the healthcare organization you would like to connect.
- Log in using that organization’s MyChart username and password.
- Follow the prompts to complete the connection.
Once linked, you may be able to view information from participating healthcare organizations within your Neighborhood Family Practice MyChart account, making it easier to manage your healthcare in one place.
Please note that not all healthcare organizations participate in account linking, and available information may vary by provider.

Payments
When an over payment is made to the organization a refund will be processed within 15 days of the date NFP becomes aware of the over payment or within 15 days of the customer requesting the refund, whichever is sooner. Refunded amounts will only be made on over payments greater than $5.00 and will be made via check.
