With MyChart, you can securely access your medical records from wherever you are, whenever you need them. Use your personal computer or smartphone to find out results of your blood test, schedule your next appointment, or check to see exactly when you received your last tetanus shot.

MyChart features:

  • Send secure messages to your provider.
  • View your test results.
  • Schedule, view and cancel appointments.
  • View current health issues, immunizations, allergies and vital signs.
  • View your current medications and request prescription refills.
  • Receive preventative care reminders.
  • Track your daily health readings and send them to your provider.

Signing up for MyChart is easy

You may sign up for a MyChart account one of two ways:

In Person. During your next visit, ask your front desk or care team member for a MyChart activation code. Select “Login to MyChart” above. Click “Sign Up Now.” Enter your activation code and complete the rest of the form. Create a username and password. NFP recommends using your first initial of your first name, followed by your last name as your username.

Online. Click the “Login to MyChart” button above, which will take you to the OCHIN* MyChart home page to request an activation code. Click “Sign Up Now.” Complete the MyChart Request an Activation Code form and select “Submit.” A one-time activation code will be emailed or mailed to you. Follow the instructions provided to you in this communication to finish setting up your MyChart account.

Recognized as one of the nation’s largest and most successful health information networks, OCHIN specializes in providing services to community health centers across the nation. When an over payment is made to the organization a refund will be processed within 15 days of the date NFP becomes aware of the over payment or within 15 days of the customer requesting the refund, whichever is sooner. Refunded amounts will only be made on over payments greater than $5.00 and will be made via check.
