Our Health Care Services

Neighborhood Family Practice provides quality, primary health care for people of all ages. Every year, nearly 19,000 patients receive quality health care in our bilingual, multi-cultural office.

New uninsured patients must schedule a visit with a financial counselor before scheduling a doctor visit. Please see Services to the Uninsured and Low Income and call us to make an appointment.

Same day appointments are available. Call our office in the morning to schedule.

Current Patients: Your health and well-being is our top priority. At Neighborhood Family Practice, you have 24-hour access to a provider and their team who can answer your health related questions. Day or night – simply call the medical office where you receive your care. For life threatening emergencies, always call 911 or go to your nearest emergency room.
Preparing for your visit:

At each visit, you will be asked to provide the following:

  • Co-payment (due at time of service)
  • Some form of personal identification
  • Proof of insurance

W. 117th Community Health Center:

North Coast Community Health Center:
Monday, Wednesday, Thursday and Friday: 8 a.m. to 5 p.m.
