Insurance, Billing and Financial Assistance

Whatever the size of your family or income, NFP provides affordable, quality health care to meet your needs. We have a team of benefit specialists and certified application counselors (approved by the Federal Government) who can help answer questions about health care benefits, assess your health insurance needs, provide information on basic health insurance issues and plans, and assist with applying for coverage and accessing benefits.

Important Medicaid Updates

As your health care provider, Neighborhood Family Practice wants to ensure that you are aware of changes that will impact your Medicaid health care coverage. The eligibility of ALL Medicaid-insured patients will be verified by the Ohio Department of Health. To determine your eligibility, Ohio Medicaid needs you to verify or update your current contact information including:

  •       Name
  •       Home address
  •       Mailing address
  •       Phone number
  •       Email address

To update your contact information with the Ohio Medicaid office, call 1-800-324-8680 or click the button below. You will need your account number when you call or username and password when you go online.

Follow these three important steps to ensure your keep your Medicaid coverage:

  1. Update your contact information – see button above.
  2. Open your mail from the Ohio Department of Health.
  3. Follow all deadlines in your letter.

Unfortunately, NFP staff members are not able to make any updates or confirm your information in the Medicaid system.

Insurance

We accept Medicaid, Medicare and most health insurance plans and offer financial assistance if you do not have insurance.

Co-pays

All co-pays are due at the time of your appointment.

Billing

If you have billing questions, please call 216.281.0872 and follow the prompts. We can set up an affordable payment plan if needed and help direct you to the appropriate resources for insurance questions. Also, we can provide you with a good faith estimate of their medical charges.

Financial Assistance

  • NFP offers financial assistance to eligible patients
  • Discounted fees are based on family size and income using the following criteria:
    • Patients with income at or below 100% of the federal poverty level pay a nominal charge
    • Patients with income between 100% and 400% of the federal poverty level receive a sliding fee discount
    • Patients with income above 400% of the federal poverty level are not eligible for a sliding fee discount
