In March 2014, Neighborhood Family Practice (NFP) celebrated the grand opening of its fourth community health center location in response to a tremendous need for primary health care in the Puritas/Bellaire community.

Shortly thereafter, patient volume began to overwhelm the small office located in Puritas Park Plaza, so NFP administrators applied for and received a Health Infrastructure Investment Program Award from the U.S. Department of Health & Human Services to offset some of the costs related to the purchase and renovation of a larger facility.

Today, the new NFP Puritas Community Health Center is housed in a 6,400-square foot free-standing building located at 14625 Puritas Avenue (corner of W. 147th Street & Puritas) that has been completely renovated and includes 12 patient care rooms – 7 more than the original location, with the capacity to serve more than 4,000 patients annually.

“The community’s response to our Puritas original location, and the relationships developed there over the past few years between patients and providers were key to NFP committing to purchasing, renovating, and opening the new, larger location,” says Megan Meister, NFP’s Director of Community Engagement. “We’re looking forward to continuing to provide high quality health care and expanded services to not only our existing Puritas patients, but also the additional patients the new larger facility will allow us to accommodate.”

The new NFP Puritas Community Health Center offers:

  • Primary health care
  • Behavioral health services (for established patients)
  • Women’s health and midwifery pregnancy care
  • Lab testing
  • Same day appointments
  • Free parking
  • Financial assistance

NFP accepts Medicaid, Medicare and most health insurance plans and offers financial assistance for those without insurance.
