Our Ann B. Reichsman and Tremont Community Health Centers are now located in a spacious, newly renovated site at 3466 St. Roccos Court in the Clark-Fulton neighborhood. The new center is in the historic Blanket Mills building.
At this location, our dedicated medical, behavioral health, and midwifery care teams are ready to welcome you. The space was thoughtfully designed with your comfort and convenience in mind and features:
- An onsite parking lot
- Easy access via the 25 and 45 bus lines
- A large community room for programs and gatherings

We are currently accepting new patients with Medicaid, Medicare, and most other insurance plans.
