Care Centered Around You…
If you are a new patient or thinking of becoming one, welcome! Please read this information so you will know how to work with Neighborhood Family Practice as your regular source for medical care – your medical home. We have convenient hours – including evening hours – and offer same day appointments.
Being your medical home means that we can see you and your family for primary health care needs, we will keep your medical records at our office, and we will make referrals if you need specialty care that we cannot provide. We can do more for you than a regular doctor's office. You will have a primary care provider that will be your main doctor or nurse practitioner for regular care. If you are sick and need to be seen sooner, you can call and get scheduled with any of our providers who have appointments open the same day. We have open appointments every day so that you can get the care you need when you need it. We encourage you to see the same provider each time in order to build a trusting and caring relationship. This will help us keep you healthy!
We welcome patients with any insurance! This includes any Medicaid plans - CareSource, Buckeye Health Plan, UnitedHealthcare, Molina, Paramount Advantage and Medicare. No one will be denied access to services due to inability to pay; and there is a discounted/sliding fee schedule available. When applicable, this statement should be translated into the appropriate language/dialect.
We accept the majority of insurances from your job or own policy. We will also work with you if you have a plan with a high deductible.
We are not able to see many patients with HealthSpan health insurance. HealthSpan has its own medical system and has a specified network.
We also welcome patients who are uninsured.

You can choose your medical provider, or one will be chosen for you when you call to make an appointment. If your insurance requires it, please let your insurance company know that you will be receiving care here.
