Our Doctors and Nurse Practitioners

Neighborhood Family Practice is a health center with a family practice approach. All of our physicians are board certified in family practice. All medical providers see the entire family (unless noted) and work to identify problems negatively impacting the health status of the family as well as the individual. Our doctors, nurse practitioners, nurse midwives and behavioral health therapists provide care that is based on the needs of each person in a respectful, confidential, and caring manner.

Erick Kauffman, MD, MPH Chief Medical Officer

Ridge Road Community Health Center

Erick Kauffman, MD, MPH received his medical degree from the University of Pittsburgh in 1997. He did his training in family practice at the University of Washington in Seattle, Washington. He came to Neighborhood Family Practice in 2000 to fulfill his service commitment with the National Health Service Corps and has made NFP his career. He enjoys seeing patients of all ages, including prenatal care. He also speaks Spanish. Dr. Kauffman is married, has three children and enjoys family and outdoor activities.

Melanie Golembiewski, MD, MPH Associate Medical Director

W. 117th Community Health Center

Melanie Golembiewski, MD received her bachelor’s degree from The Ohio State University and her medical degree from Wright State University. She completed her family medicine residency program at University Hospitals and a preventative medicine fellowship with NFP. Dr. Golembiewski has a strong interest in maternal and neonatal care, global health and geriatrics.

Kathy Chilton, APRN-CNP

Ridge Road Community Health Center and Tremont Community Health Center

Kathy Chilton joined Neighborhood Family Practice in 2019. A family nurse practitioner, she earned her bachelor of science in nursing and master of science in nursing from the University of Texas School of Nursing. Kathy has over 25 years of experience as a nurse and a decade of experience as a nurse practitioner. She has provided primary care services to adult and pediatric populations managing acute and chronic illnesses.

James Diekroger, MD

Puritas Community Health Center

James Diekroger, MD graduated from the Ohio State University College of Medicine in Columbus, Ohio. He completed his residency training at the University of Rochester Highland Hospital in Rochester, New York. His areas of interest include prevention and education, as well as integrative medicine. He speaks French and a little Spanish. He is married and enjoys disc golf, as well as movies and books, especially those that involve science fiction.

Amy Ebbitt, APRN-CNP

Detroit Shoreway Community Health Center

Amy Ebbitt, family nurse practioner, received her bachelor's degree from Wheaton College in Anthropology and a master of science in nursing from Case Western Reserve University. Amy has worked at Neighborhood Family Practice since 2009. Prior to working at NFP, Amy worked in Infectious Disease and Women’s Health at University Hospitals. Amy is committed to ensuring healthcare access for all. Amy enjoys spending time with her family in her free time.

Katie Edwards, APRN-CNP

Puritas Community Health Center

Katie Edwards received a master of science in nursing from Rush University in 2012. Katie has more than 10 years of clinical experience and works with Neighborhood Family Practice as a family nurse practitioner.

Chad Garven, MD, MPH

Tremont Community Health Center and Ridge Road Community Health Center

Chad Garven, MD completed his undergraduate degree at Marquette University. He earned a master of public health and his medical degree from Wright State University. Dr. Garven completed his family medicine residency in 2015 at The Medical University of South Carolina and returned home to Cleveland to practice family medicine at Neighborhood Family Practice.

Chuck Garven, MD

North Coast Community Health Center

Chuck Garven, MD, earned his medical degree from Case Western Reserve University and his undergraduate degree from Cleveland State University. He completed family medicine residency at The Medical University of South Carolina. Chuck enjoys providing care to families with people of all ages. A native of Cleveland, Chuck spends his free time with his growing extended family and enjoys traveling.

Heidi Gullett

Heidi Gullett, MD, MPH

Ridge Road Community Health Center

Heidi Gullett, MD, MPH earned her bachelor’s degree from Denison University, her medical degree from Wright State University and a Master of Science in Public Health from Portland State University. She is board certified in both family and preventive medicine. Dr. Gullett has spent most of her career working in health centers that serve low income and vulnerable populations.

Donna Kelly, APRN-CNP

Puritas Community Health Center

Donna Kelly joined Neighborhood Family Practice in 2016 as a family nurse practitioner. Donna received both her master of science in nursing and bachelor of science in nursing from Ohio University. With her degrees, she has continued her career of helping those in need, especially the homeless. Donna’s passion for her work can especially be seen through her post-graduate residency at Louis Stokes Cleveland Veterans Affairs Medical Center.

Lauren Kreiger MD, MPH

Ridge Road Community Health Center

Lauren Kreiger MD, MPH received her bachelor's degree and masters of public health degree from the State University of New York at Albany and her medical degree from the University of Vermont. Lauren is board certified in family medicine and completed her residency training at University Hospitals Cleveland Medical Center. She has a particular interest in maternal and child health. Outside interests include hiking, biking, yoga and vegetarian cooking.

Joseph Labastille, MD

Detroit Shoreway Community Health Center

Joseph Labastille, MD received his medical degree from the State University of Haiti. He has over 30 years of experience in family medicine. Dr. Labastille also sees patients at Ridge Community Health Center.

Tina Laisure, APRN-CNP

In 2016, Tina Laisure, a family nurse practitioner, joined the NFP staff. Her credentials include a master of science in nursing from Ursuline College and a bachelor of science in nursing from Regis University. She has experience as a family nurse practitioner and specializes in family medicine. Tina enjoys her job because she loves the challenge and the opportunity to make a difference in someone’s life.

Joannah Lynch, APRN-CNP Associate Medical Director

Ridge Road Community Health Center

Joannah Lynch, family nurse practitioner, earned her bachelor’s degree from the University of Cincinnati and a master of science in nursing from Case Western Reserve University. She is the associate medical director overseeing nurse practitioners at Neighborhood Family Practice. She also oversees refugee services. Joannah is fluent in Spanish and enjoys providing the highest quality, culturally competent nursing care to all of her patients.

Natalia Parkanzky, APRN-CNP

Ridge Road Community Health Center

Natalia Parkanzky, family nurse practioner, received her Bachelor of Science in Clinical Laboratory Science from Miami University and a Master of Science in Nursing from Case Western Reserve University. Natalia grew up and currently resides on the west side of Cleveland. She enjoys encouraging patients of all ages to maintain a healthy lifestyle with daily exercise and a balanced diet. She speaks Spanish and Ukrainian. Her outside hobbies include running, baking desserts, and dancing and singing with her children.

Aleksandra Rabinovich, APRN-CNP

W.117th Community Health Center

Aleksandra Rabinovich, family nurse practioner, holds a master of science in nursing, and received her education and training as a family nurse practitioner from The Ohio State University College of Nursing. Aleksandra has professional experience as a family nurse practitioner in both outpatient and inpatient settings, and has previous experience as an emergency nurse in an urban trauma center. She has a passion for preventive care. Aleksandra speaks fluent Russian, and in her free time, she enjoys salsa dancing and spending time with her family.

Ann Reichsman, MD Senior Medical Advisor

Ridge Road Community Health Center

Ann Reichsman, MD is recognized as leader in the delivery of high quality community health care. Dr. Reichsman graduated from Antioch College with a Bachelor of Science degree in Biomedical Sciences and a minor in Dance. She received her medical degree from the SUNY Downstate Medical Center and did her residency in family medicine at MetroHealth Medical Center. Dr. Reichsman is board certified in family medicine. She is assistant clinical professor at the School of Medicine, Case Western Reserve University. In 1996 Dr. Reichsman was recognized as the Family Physician of the Year by the Ohio Academy of Family Physicians. In 2008 Dr. Reichsman was named the Physician Health Care Hero by Crain's Cleveland Business.

Bart Rog, MD, MPH

Ridge Road Community Health Center and Puritas Community Health Center

Bartlomiej (Bart) Rog, MD, joined Neighborhood Family Practice in 2018 after completing his family and preventive medicine residency at University Hospitals Cleveland Medical Center. Bart earned his doctor of medicine from Jagiellonian University Medical College in Krakow, Poland. Bart also earned a master of public health from Case Western Reserve University. He is fluent in Polish and enjoys running, skiing and cooking in his free time.

Jennifer Sobrowski, APRN-CNP

Tremont Community Health Center

Jennifer Sobrowski joined NFP in 2017 as a family nurse practitioner. Jennifer earned her master of applied sciences in nursing from the University of Memphis and her bachelor of science in nursing from Cleveland State University. She has experience various clinical, family medicine and behavioral health settings. Jennifer dedicates some of her free time to rescuing and fostering animals to find them loving homes. Jennifer also sees patients at Ridge Community Health Center.

Teegan Wightman, APRN-CNP

North Coast Community Health Center

Teegan Wightman, APRN-CNP, earned her master of science in nursing from Case Western Reserve University. Teegan’s clinical areas of interest include preventive care, women’s health, chronic disease management and global health. She has even completed several medical missions in Guatemala. In her spare time, Teegan enjoys exploring new restaurants in Cleveland, listening to live music and spending time with her two pugs.

Anne Wise, MD, MPH

Detroit Shoreway Community Health Center

Anne Wise, MD received her medical degree from Temple University School of Medicine. She completed her residency in family medicine at St. Francis Hospital in Hartford, Connecticut, followed by a fellowship in Occupational and Environmental Medicine at the University of Connecticut Health Center in Farmington. Dr. Wise has practiced at Neighborhood Family Practice since 1995. Fluent in Spanish, her other interests include prison medicine, immigration policy, advocating for national health insurance and mystery novels (lots of them!)

Sasha Yurgionas, MD received her bachelor’s degree from Oberlin College and her medical degree from the Latin American School of Medicine. She completed her family medicine residency at MetroHealth in 2015 and now works at Neighborhood Family Practice’s W. 117 community health center.)
