Notice of Patient Privacy Practices [HIPPA]

Our Health Care Services:

Neighborhood Family Practice provides quality health primary health care for people of all ages. Every year nearly 12,000 patients receive quality health care in our bilingual, multicultural office.

We welcome Medicaid, Medicare, most health insurance, including many HMOs, and we offer a sliding fee scale for uninsured patients.

New uninsured patients must schedule a visit with a financial counselor before scheduling a doctor visit. Please see 'Becoming our Patient' and call us to make an appointment.

All patients:

  • Co-payments must be paid at the time of service
  • If you have an insurance card, please bring it to each visit

You can probably see a doctor today; we offer same day appointments.

Call our office in the morning to schedule.
