Patient Advisory on Sliding Fee Scale Increase

Becoming our patient:

If you are a new patient or thinking of becoming one, welcome! Please read this information so you will know how to set up Neighborhood Family Practice as your medical home. 

Being your 'medical home' means that we can see you and your family for primary health care needs, we will keep your medical records at our office, and we will make referrals if you need specialty care that we cannot provide. We operate like a private doctor's office. You will have a primary care provider that will be your main doctor or nurse practitioner for regular care. If you are sick and need to be seen sooner, you can call and get scheduled with any of our providers who have appointments open the day you call.


We accept almost every commercial insurance.

Note: We are not able to see patients with Kaiser Permanente health insurance -Kaiser is its own medical system and has a specified network.

We welcome patients with Medicaid HMOs (Caresource, Anthem, Wellcare, etc.) and Medicare.

We also welcome patients who are unisured (those who do not have health insurance coverage).


Patients with Medicaid, Medicare, and commercial insurance:

Call us to set up your medical visit [216-281-0872 for Ridge Road, 216-334-2800 for Tremont]. Next, call your insurance company and switch your primary care provider to the medical provider you will be seeing. You can choose your medical provider, or one will be chosen for you when you call to make an appointment.  All our providers accept new patients. 

Patients who are uninsured:

  1. Call 216-281-0872 to set up an appointment to meet with the Financial Counselor. 
  2. You will need to bring materials to be put onto our sliding fee scale [please see below]. You will then be 'rated' for a specific co-payment amount for each visit. This 'rate' will be the amount you need to pay for each visit. 
  3. After you have met with the Financial Counselor, you can make an appointment for your medical visit. 

We encourage you to think of this process like you would with health insurance—you want things to be in order before you get sick so you have a place to go when you dont' feel well, and everything is set up for you.

Eligibility for our sliding fee scale is determined by household and household size. We determine your income by looking at the ways you are supported, even if you are not working. When you call our offices and explain your situation, they will tell you what information you will need to bring. The information needs to be as recent as possible. It may be any of the following:

  • Household pay stubs for the past month
  • A recent utility bill, preferably phone bill for proof of residency (we need a valid address for billing purposes)
  • Valid state ID
  • Child support letter
  • Federal Tax Return for previous year
  • Workers Compensation Benefit Letter or Bank statement showing deposit
  • Notarized statement of self-employment earnings if self-employed
  • Social Security letter or check

Work that needs to be sent to a Lab for testing:

Lab charges are separate from Neighborhood Family Practice. The company that administers lab work takes into consideration what sliding fee scale a patient is on and will charge a discounted rate as long as a patient has completed the sliding fee process with Neighborhood Family Practice.
