Medical Home Project

Are you in your last trimester of pregnancy or

do you have a baby that is less than 4 months old? 

Do you want to improve your families’ health?

Do you want to participate in a program that ensures your baby a healthy first year?

If you answered ‘yes’ Neighborhood Family Practice invites you to participate in our Medical Home Program!

 

This program includes:

  • Information on community resources that are available to you
  • The chance to have all of your questions answered
  • The opportunity to express your feelings about the care you receive at NFP
  • Baby supplies at each visit, such as diapers, bottles, onesies, baby medical and bath kits, a diaper bag and more!
  • Two $25 gift certificates to Target for completing two surveys

 

You will be asked to:

  • Meet with NFP’s Patient Advocate after every well child visit for approximately one 1/2 hour through December, 2008
  • Fill out a survey at the beginning and end of the program
  • Talk about your concerns and questions with the Patient Advocate
  • Discuss different goals you have in mind for your family, and the type of resources you need to reach them

Westside Women's Health Consortium

The Westside Women's Health Consortium was intiated

with funding for Women's Health Month from the Ohio

Department of Health, Bureau of Health Promotion and

Risk Reduction, Sexual Assault and Domestic Violence Prevention Program, with the continuation support

provided by the Community West Foundation. 

The Consortium is open to anyone working with

women on Cleveland's west side.

Meetings are held at NFP's office at 3569 Ridge Road,

the third Friday of every month from 9-10AM. For more
